My game isn’t saving properly or my game save disappeared, what should I do?

Also, please include the following information in the first email:
1.) What platform are you using? (PC, XBOX 360, PS3)
2.) Does the issue arise at the same place in the game or at different places?
3.) Are you making sure the game saves all the way before turning off your machine, quitting or logging off?
4.) What was the last thing you did in the game?
5.) Are you able to repeat the issue? What steps did you take to repeat the issue?
