I entered the code for my Onion Helm in-game item, but it does not appear in my character's inventory.
If you have created a character prior to entering the code for the Onion Helm, the helm will not be available for that character. The Onion Helm can only be used by characters that were created after the code had been entered. Once you create a new character after having entered the code, you will see the Onion Helm in your inventory.
